University of Benin (UNIBEN) Teaching
Hospital School of Nursing (UBTH) Admission Application Form for 2014 is
out. Members of the public are hereby informed that the UNIBEN Nursing
form (UBTH) for 2014 admission obtained and you are to address to the
principal.

Entrance Examination For The 18 Month Course
Applications are invited from suitably
qualified candidates for entrance examination for admission into the
above school of Nursing in March 2014 for Eighteen (18) months (Post
Registration) Nursing program leading to the award of Registered Nurse
Certificate (RN).
ENTRY QUALIFICATIONS FOR UBTH School of Nursing:
1. Candidates must not be less than 18 years old.
2. Must possess the following:
WAEC/GCE (O/Level) NECO with five (5)
credits in not more than two (2) sittings. Subjects should include
English, mathematics, chemistry, biology and physics
b) senior secondary school certificate (SSCE) in the relevant subjects at credit levels as in (a) above
c)candidates must also possess registered midwifery certificate (RM) or Registered psychiatric Nurse Certificate (RPN).
UBTH ADMISSION APPLICATION FORM 2014:
Application forms should state age,
state of origin, local government area, schools attended with
qualifications obtained and addressed to the principal.
COLLECTION OF APPLICATION FORM
Application forms to be submitted for
the 2014 University of Benin Teaching Hospital School of Nursing
admission should state age, state of origin, local government area,
school(s) attended with qualifications obtained and addressed to the
principal.
COLLECTION OF APPLICATION FORM
Application forms are obtainable from
the office of the Deputy Director Nursing Education/Principal with a
non-refundable fee of N5,500.00 (Five Thousand Five Hundred Naira) only.
this should be made payable through teller payment to , UBTH Training
School (First bank of Nigeria Plc, Ugbowo, Benin City).

A separate payment of N200 to nursing
and midwifery council committee, Edo state, account no: 2050003858 or
(1112050003858) first bank of Nigeria plc, Ugbowo.
Candidates are to submit four (4)
recent colour passport photographs (taken in preceding two weeks) while
returning the completed application forms.
